Elder Care Management services include:
- Assessment of safety, medication, nutrition, care needs, mental health and emotional well-being
- Development of a care plan to meet the needs of the elder and his/her family
- Connection with community resources
- Help in evaluation and choice of appropriate housing plus assistance with a move
- Advocacy, including accompanying elders to medical appointments
- Evaluation and coordination of home care services
